Dayton Has Sole โ Shoe Discount Program Guidelines
๐ Please read the following information carefully. To be considered for a discounted pair of supportive shoes, you must fully complete the Shoe Discount Application and return it along with income documentation.
This program offers discounted shoes to individuals who meet income-based eligibility guidelines. Discounts are based on household size and total gross income.๐จโ๐ฉโ๐งโ๐ฆ Definition of Household
"Household" includes all individuals related by birth, marriage, or adoption who live together. This may also include extended family members such as grandparents, grandchildren, aunts, or uncles residing in the same household.๐ต Definition of Income
Income includes (but is not limited to):- Employment wages or salaries
- Social Security, SSDI, SSI
- Unemployment or Workers Compensation
- Public assistance or SNAP
- Retirement, pension, or survivor benefits
- Rental income, alimony, or child support
- Self-employment income
๐ Required Documentation (provide at least one of the following):
- Prior year W-2 or 1040/1040A tax return
- Two most recent pay stubs
- Form 4506-T (if taxes not filed)
- Social Security or disability benefit letter
- SNAP/Food Assistance approval letter
- Unemployment compensation stubs
- Bank statements for fixed income
- Letter of reference from a 501(c)(3) nonprofit (e.g., shelter, church)
- Self-employed individuals must submit a detailed summary of income and expenses for the past three months

Return the completed application and documentation to Dayton Has Sole by:- Uploading via our online application portal
โ What Happens Next
- Once your application is completed and proof of income is submitted it will be reviewed.
- You will be notified via email if your application is approved, denied, or if more information is needed.
- Once approved, you will receive confirmation and a list of scheduled events for your shoe-fitting appointment as well as your reduced rate.
- Discount approval is valid for 12 months, unless your financial situation changes significantly.
